The City of Hoover Municipal Court provides the following forms for your convenience. Please note that forms may change without notice. While we will make every attempt to keep this list accurate, it is your responsibility to make sure you are using the correct form.

FORM NAME (Click to Download)	PURPOSE
Request for Access to Public Court Records	Use this form to request copies of records that are normally made available to the general public.
Speedy Trial Petition Form	This form must be filled out if you have petitioned the court for relief under the speedy trial clause or similar provision of law.
Indigency Status Form	Used to request a court appointed attorney.

To obtain Accident Reports or Incident/Offense Reports, please contact the Hoover Police Department.
